Dear Sir:-
We beg to acknowledge yours under date of the 6th inst., relative to Policy No. 525540, enclos-ing order form No.2 advising that you desire Novem-ber 1912 cash dividend of $259.02.
We have forwarded same to the Company for their attention.
Said policy became paid-up on November 22nd, 1912, for $1,000.00. If you desire dividends there- under declared annually instead of very five years, as provided in the contract, the enclosed request for annual dividends should be dated and signed, each sig-nature being witnessed, and returned with the policy in the enclosed stamped envelope.
