Instructions to Financial Secretaries
In order to expedite a settlement of every claim for death benefits, the Certificate of Membership held by the deceased member must be made a part of the Proofs of Death as required by Section 83 of the Constitution, Laws and By-Laws of the Association. The Financial Secretary of the Camp is requested to obtain such certificate and attach the same to the Proofs of Death and shall execute and deliver to the beneficiary the following receipt for same.
If the claim is approved, a warrant for the amount approved will be sent to the Financial Secretary, who shall deliver the same to the beneficiary or beneficiaries and shall take up and destroy said receipt for the certificate. If the claim is not approved, the certificate of membership will be returned to the Financial Secretary, who shall return it to the beneficiary on surrender of the receipt and forward it to the Claim Department at Omaha, Nebraska.

Received of Roxie Wilson Beneficiary Certificate No. RW848936L, issued to WM.Z.M. Wilson deceased, the surrender of said certificate being for the purpose of making it a part of the Proofs of Death.
Should the claim under this certificate be approved by the President of the Association, a warrant for the amount approved, with receipt attached, will be forwarded to the Financial Secretary for delivery to the beneficiary.
Should the claim under this certificate be rejected, it is agreed that the certificate will be retuned to Financial Secretary, upon application, to be returned to the beneficiary.
